• The Policy Number
• The Insured Person’s Name
• The Insured Person’s Location
• The Insured Person’s Date of Birth
• Policy Holder’s Name (if different)
• Nature of the Incident
• Next of Kin (if applicable)
• If medical treatment has commenced, the name of the medical facility and their contact details.
• Any other relevant information that may affect our response
• Medical Assistance and Incident Response is provided by Inherent Risks.
